What To Expect  

The art of health and functional wellness requires a complete understanding of all the factors that may contribute to your health and well-being, so open dialogue with your doctor is encouraged. At your first office visit, you and your doctor will outline your past and present health goals and concerns. Your doctor will document a thorough medical history for you. Recommended diagnostic laboratory tests may be ordered to provide a complete understanding of your health. If you have had recent lab work,within the past 12 months, please bring a copy of the reports with you. At your first visit, we spend a full hour with you – please arrive with your paperwork completed and if you are married or in a domestic partnership relationship, it is ideal if your spouse is present.

New Patient Forms

When you schedule your first office visit, you will receive instructions on how to download New Patient forms from our website. Please ensure that these are completed fully prior to your appointment.

Return Visits

Your doctor will determine when you should schedule any return visits based upon diagnosis, treatment plans, health goals, and appropriate re-evaluations. Return visits will either be a private appointment with your practitioner, which typically last 30 minutes, or a Wellness Workshop class, which typically last 45 minutes to 1 hour. Please note any observations, symptoms that have improved or worsened, and questions you would like to address since your previous visit.

Payment

Payment in full is due at the time of service. Our office accepts cash, checks, Visa, and MasterCard. We also accept payment from your Health Savings Account (HSA) or Flexible Spending Account (FSA). We do not bill insurance companies directly, but upon your request we can provide you with a copy of your complete invoice so that you may submit it for reimbursement. We also provide medical financing programs for patients who find monthly payments more convenient.

Cancellations

Apex Functional Medicine has a two (2) business-day cancellation policy. If you are unable to make it to your appointment, please give us at least two business days’ notice so we may offer the appointment time to someone else. Without proper notice, a cancellation charge will apply.
